Registering a Death the steps involved:

In order to register a death it is necessary to collect the Death Notification Form (ie Hospital Death Cert) from the Hospital. (It is normal procedure for the Hospital to notify the Next of Kin in writing when this form is available for collection)

Once the Next of Kin has been notified that the Death Notification Form is available, it may be collected from the Patient Services Department, Naas General Hospital (045 849514). Office Hours are Monday-Friday 9am-1pm and 2pm-5pm. 

The Death Notification Form may be collected by the Next of Kin or a nominated relative.

Part 2 of the Death Notification Form must be completed, before it is then brought to any civil registration office to officially register the death.

The registering of a death can take place up to 3 months from date of death.
