Request my records

You can apply for a copy of your patient records by completing a subject access request (SARs) application form. You can access this by clicking on the following link:- https://www.hse.ie/eng/gdpr/sarsform.pdf.

This form can be submitted along with a copy of your photographic identification to requestmyrecords.naas@hse.ie and it will be processed within one month under GDPR legislation.

You may also forward your request in writing by post to the Quality, Risk and Patient Safety Department, “Request Records Section”, Naas General Hospital, Naas, Co. Kildare.

A request may also be made with your permission by a third party (e.g. your legal representative, a family member or the Gardaí). A written authority signed by you should accompany the third party request.

Requesting a deceased person’s records

Please note that requests for a deceased patient’s information must be made under the Freedom of Information (FOI) Act 2014. You can make an FOI request by completing an FOI request form. You can access this by clicking on the following link:-

This form can be submitted along with a copy of your own photographic identification together with documentary evidence of your relationship to the deceased e.g. birth cert (if son/daughter of deceased) or marriage cert (if spouse) to requestmyrecords.naas@hse.ie and it will be processed within 20 working days under FOI legislation.

You may also forward your request in writing by post to the FOI Decision Maker, Quality, Risk and Patient Safety Department, Naas General Hospital, Naas, Co. Kildare.

Registering a Hospital Death the steps involved:

In order to register a death it is necessary to first obtain the hospital’s Death Notification Form. It is normal procedure for a staff member from the Quality Risk and Patient Safety Department of the hospital to notify the primary contact of the deceased patient when this form is available. The Death Notification Form will then be sent by registered post to the family. Arrangements can be made to facilitate collection of the form if it is more convenient.

Part 2 of the Death Notification Form must be completed by the family. They should then contact their nearest Civil Registration Office to make arrangements to register the death and obtain the official Death Certificate. Some registration offices work on an appointment system. Contact details of registration offices will be provided by the hospital to the family together with the Death Notification Form.
