Oncology Nurse Co-ordinator

The Oncology Nurse Co-ordinator is part of the multi-disciplinary team and plays a key role in the linkage between various hospitals, departments and relevant Professionals in the hospital and in the community. Central to the role is improving the co-ordination of treatment and management of patient care. The Role is also to assist in meeting the psychological emotional and information needs of patients with cancer.

Service Provided

  • Co-Ordination of Cancer Service.
  • Provide Information, Advice and Support.
  • Referral to Self Help and Voluntary Groups.
  • Contact person for GPs, and Community Services.
  • Staff Education.
  • Telephone Advice.
  • Attend Weekly Medical Oncology Out-patient Clinic.
  • Health Promotion.
  • Co-Ordination of Oncology / Haematology Day Services.
  • Liase with Palliative Care Services
  • Head of Services

Clinical Nurse Specialist Diabetes

Objectives:

  • Education of newly diagnosed type 1 and type 2 clients with diabetes.
  • Assessment and education of clients with poor metabolic control.
  • Advice and education of clients commencing insulin either as a new type 1 diabetic or a failed type 2 diabetic.
  • Ongoing assessment, monitoring and evaluation.
  • Education and training of staff.
  • Telephone advice service for ongoing assistance and help.
  • Assist at Consultant led diabetic clinics twice monthly.

Patients who are attending any of the Hospital Consultant's Clinics may be referred to the Diabetes Service for education.A written consultation referral is requested for each client which should be completed by medical or surgical staff and sent to Diabetes Education Office, 2nd Floor, OPD. Referral forms for Diabetic education can be obtained from Jackie or Joanne if required.
