Diabetic Day Centre

The Diabetic Day Centre is a nurse led service. Diabetes nurses provide:

  • Nurse led diabetes clinics both face to face and virtual review
  • Avoiding admission by providing a walk-in, phone support and an electronic service
  • Advice and support to primary care providers, like GP's, nursing home staff, Public Health Nurses or other allied health professionals
  • Education, review and support of persons with diabetes attending the Consultant diabetes clinics
  • Inpatient assessment and review of persons with diabetes
  • Support to the multidisciplinary team. We help to facilitate safe inpatient care, and timely discharge with appropriate follow up care
  • Initiation of persons with Diabetes on Continuous Glucose monitoring (CGM)
  • Supporting persons with diabetes using CGM to optimise glycaemic control
  • Supporting persons with diabetes using Insulin pumps to manage diabetes
  • Education and training of staff
