The Diabetic Day Centre is a nurse led service. Diabetes nurses provide:
- Nurse led diabetes clinics both face to face and virtual review
- Avoiding admission by providing a walk-in, phone support and an electronic service
- Advice and support to primary care providers, like GP's, nursing home staff, Public Health Nurses or other allied health professionals
- Education, review and support of persons with diabetes attending the Consultant diabetes clinics
- Inpatient assessment and review of persons with diabetes
- Support to the multidisciplinary team. We help to facilitate safe inpatient care, and timely discharge with appropriate follow up care
- Initiation of persons with Diabetes on Continuous Glucose monitoring (CGM)
- Supporting persons with diabetes using CGM to optimise glycaemic control
- Supporting persons with diabetes using Insulin pumps to manage diabetes
- Education and training of staff
