GP Liaison Clinical Nurse Specialist II

The GP Liaison Clinical Nurse Specialist II (CNS II) focusses on promoting clear and relevant communication between the hospital and primary care providers. Particularly in the areas of patient referral and discharge.

The GP Liaison CNS II offers a single point of contact to GP's for:

• Clinical queries
• Enquiries about patients referred to the Emergency Department (ED)
• Accessing reports or results
• Post discharge patient follow-up
• Information on critically ill patients admitted through the ED
• Providing critically ill patients transfer to another hospital
• Follow up with patients who did not wait to be seen or left against medical advice
• Guidance on referral pathways
• Guidance on services and specialties

Office Hours

Monday, Tuesday, Thursday 08.00-16.00

Wednesday, Friday 08.00-13.00

(Except bank holidays)
